It seems the trouble with 1.1.1 is that it doesn't allow any ADDITIONAL files, but, as reported, the ringtones added by iToner still exist, they just are no longer referenced by the database and that database gets wiped after every sync.
My suggestion (as a non-programmer) is that there are several ringtones I will never use. Would it be possible to REPLACE one of the unwanted ringtones? It would be named the same, but would just be a different file and the iPhone would think it's playing the correct file.

I second this idea... If this approach would work, I too would be willing to use a "faked out" existing ringtone... So sad they broke it.

There is a ringtone workaround against iPhone 1.1.1 but it only works with iTunes 7.4.0.
I hesitate in posting the link to the workaround since it uses another 3rd party ringtone application. The gist of it uses the "old" trick of renaming your AAC files to the "m4r" extension, adding them to iTunes 7.4.0, syncing your iPhone, renaming the ringtone files in your iTunes Library back to "m4a", using this other 3rd party app to add the renamed m4a files back into iTunes (so you end up with 2 sets of your custom ringtones), and then syncing your iPhone again. Only one set of the ringtones appears on the iPhone, but the ringtones play and can be assigned to contacts.
It's still a bit of a chore to use this workaround, but at least it will give the Ambrosia developers something to look at and research.

Yes, I was able to the workaround functioning. One key is that you have to perform the sync process twice. Once while the ringtone filenames are .m4r, and then after the name change to .m4a and using the other 3rd party application. I don't want to advertise another 3rd party app on Ambrosia's board but if you do a search on MacRumors' forums, you'll find it.
I'm hoping that Ambrosia is able to update iToner as I'd much prefer that as a solution over a multi-step workaround and an older version of iTunes.

Does it actually matter WHICH "other" program you use? I tried it with what I think is a different one (the one that sounds like it should have Arnold Schwartzenneger as a spokesman) and couldn't get this trick to work. The duplicate ringtones never showed up in iTunes. On the other hand, I "downgraded" to 7.4.0 -- perhaps that makes a difference?

Apparently, yes, it does. Either that or I just wasn't doing it right the first time. With 7.4.0 and the "other" program, I did get this to work. But *only* with my first file I tried. None of the others worked...

Still looking forward to a better solution.

FWIW, I've figured out that the reason only one of my files worked with this hack seems to be length related. If I trim the file down it works fine. Seems 1MB is about the limit here.
